Death Claim

Ensure all relevant documents below are completed:

  • Death Claim form
  • Medical Attendance Certificate pertaining to your condition. Please refer to your servicing agent (if any) or go to our nearest branch for assistance to complete additional requirements.
  • Certified True Copy of Death Certificate (by PruBSN or Prudential Assurance Malaysia Berhad branch personnel or Business Development Executive/ Manager)
  • Certified True Copy of Deceased's IC/ Birth Certificate
  • Certified True Copy of Claimant's IC
  • Respective documents in the following table:

Submit the completed claim form (s) with required documents to  the nearest PruBSN or Prudential Assurance Malaysia Berhad branches  or servicing agent if you have one.

  2. Ensure your signature on the Claims forms tallies with the one in the records.
  3. Confidential Medical Certificate is to be completed at the claimant's own expense.

  • CrisisCover, Spouse Contributor or Parent Contributor claim forms
  • Medical report pertaining to your condition (where applicable).
  • Other supporting test/ laboratory report - biopsy, MRI, scan and investigation results (where applicable)

Excess Medical Reimbursement Amount from Employer/ Other Insurers or Takaful Operators

Ensure all relevant documents below are completed.

  • Medical Claim form.
  • Certified True Copy of medical report pertaining to your medical condition.
  • Other supporting test/ laboratory report - biopsy, MRI, scan and investigation results (where applicable).
  • Certified True Copy of Bills & Itemised Bills.
  • Original receipts or Certified True Copy of receipts.
  • Letter from employer/ other insurers or Takaful operators indicating the amount paid.

Submit the completed claim form (s) with required documents to  the nearest PruBSN or Prudential Assurance Malaysia Berhad branches or servicing agent if you have one.
