For Accidental/ Unnatural Death:

  • Death Claim form
  • Medical report pertaining to the cause of death. Please refer to your servicing agent or go to our nearest branch for assistance to complete additional requirements (if required).
  • Original Certificate Document
  • Certified True Copy of Death Certificate (by PruBSN or Prudential Assurance Malaysia Berhad branch personnel or Business Development Executive/ Manager)
  • Certified True Copy of Deceased's IC/ Birth Certificate
  • Certified True Copy of Claimant's IC
  • Respective documents in the following table:

  • Certified True Copy of Claimant's IC

  • Total & Permanent Disability BenefitTotal & Permanent Disability Claim (Child) forms
  • Medical report pertaining to your condition. Please refer to your servicing agent or go to our nearest branch for assistance to complete additional requirements (if any).
  • Original Certificate Document
  • Other supporting test/ laboratory report - biopsy, MRI, scan and investigation results (where applicable)

  • Certified True Copy of medical report pertaining to your medical condition.
  • Other supporting test/ laboratory report - biopsy, MRI, scan and investigation results (where applicable)
  • Certified True Copy of itemised bills
  • Certified True Copy of original receipts
  • Letter from employer/ other insurers or Takaful operators indicating the amount paid
