Refer to the Medic Plan Appendix and the Occupation Class Table to understand the contribution amount that suits your budget based on your occupation and selected coverage.
Submit and completed Proposal Form and Additional Customer Declaration Form via email to nb.direct.channel.support@prubsn.com.my. Upon receiving your completed forms, we will provide a PDS that reflects your deductible plan choice and the actual rates via email prior to enrollment.
Confirm enrollment by replying to the email from us and attach the acknowledged PDS, confirming that you have received and read the PDS. Upon successful enrollment, we will send you the Takaful Certificate with PDS and other supporting materials in digital format.
No personal data will be collected when you click the link.
Free Look Period: You may cancel Your Certificate within 15 days from the date Your Certificate is delivered to You. Your Certificate is deemed to have been delivered to You on the date You downloaded the e-Certificate from Your customer portal. If You do not download the e-Certificate, it shall be deemed delivered after the third e-notification of the e-Certificate being made available on Your customer portal.
