The contribution under this scheme is partially sponsored by the Government of Malaysia, PruBSN and the Master Certificate Holder. Need to pay a one-time takaful contribution of RM10 to partially cover the Upfront Wakalah Charge for the management expenses to PruBSN. |

How to Make a Nomination
Why is it important to make a nomination?

Nomination is important to ensure that your loved ones will receive the protection as intended by you and that PruBSN can pay certificate benefits out without delay. The nominee can be either an executor (Wasi) or a beneficiary under a conditional Hibah (Gift).

Without a nomination, the certificate benefits will be paid out to your lawful executor or administrator and distributed in accordance with the applicable distribution laws. This process may take a while.

It is essential to inform your nominee about the certificate benefits and nomination.
Who can I nominate for my certificate?

The nominee must be an individual. Nomination of an organisation is not allowed.

If your nominee is under 18 at the point of claim: the benefits will be paid to a surviving parent or proper claimant, i.e., legal guardian, subject to the relevant legal evidence/supporting documents.
How to make a nomination?

You can nominate your nominee(s) by completing the nomination form with a witness and submitting it via the following channels:

Walk in at any PruBSN or Prudential branches; or

E-mail to customer.microtakaful@prubsn.com.
• Or any other channel PruBSN may make available to you from time to time.
• The nomination form can be downloaded here, or obtained from:1. Any PruBSN or Prudential branches; or
2. The e-mail you receive upon issuance of your Certificate.
How to Submit Claims?
For a claim on Death or Daily Government Hospital Allowance Benefit (Due to Accident):

The claimant may submit the completed claim form(s) with supporting documents to any PruBSN or Prudential branch.
What are the documents needed for claim?
For a claim on Death Benefit:
The claimant is required to submit the following:

Death Claim Form - signed by the claimant (the person who is making a claim)

Medical Attendance Certificate - signed by the doctor who last treated the deceased
-
Certified True copy of Death Certificate - to be certified by PruBSN branch staff or the Executive/Business Development Manager

A copy of Claimant's MyKad (if there is no nomination)
-
A copy of Deceased's MyKad
-
E-Credit Form completed by the nominee/claimant
-
Police report, copy of driving license, other supporting tests/Laboratory Reports: Biopsy, MRI, scan, and investigation results (if involved in an accident)
All relevant claim forms can be downloaded here or obtained at any PruBSN or Prudential branch. Claim forms can also be requested by sending an e-mail to customer.microtakaful@prubsn.com or by contacting the PruBSN Customer Service line at 03 2775 7188 (Monday to Friday, 8.30 a.m. - 5.15p.m.).
For a Daily Government Hospital Allowance Benefit (Due to Accident) claim:
The claimant is required to submit the following:

Police Report, a copy of driving license

E-Credit form that has been completed by the nominee/claimant

Copy of the certified discharge letter

Other supporting tests/Laboratory Reports: Biopsy, MRI, scan, and investigation results (where applicable)

How long does it take to process a claim?

For death claims, payment will be made within five (5) working days after all the required documents are received.

For claims related to the Daily Allowance for Admission to Government Hospitals (due to accident), processing and payment will be completed within seven (7) working days after all the required documents are received.
